Stellar Inc. is an independent consulting practice founded in 2007, focused exclusively on the clinical, regulatory, and reimbursement realities of post-acute care. We work at the intersection where clinical documentation, PDPM classification, and compliance meet — the place where revenue integrity is either protected or quietly lost.

Toni has spent three decades reading those lines — as a nurse and as a former CMS surveyor — and she holds the same exacting standard on both sides of them: the care that is given, and the record that has to prove it.

That precision isn't academic — in documentation, detail is money. A single line, captured well or captured poorly, is the difference between what a facility earns and what it quietly loses. Practical by nature, Toni doesn't just point out the gap; she builds the system that closes it — turning close attention to the record into a method others can run.
